Discharge Planner (part-time) - Worcester, MA
Description: Twenty hours weekly in the AM. Assist patients with evaluation of risk factors and the development of discharge plans.
Qualifications: R.N. Knowledge of high risk medical/psych. conditions of substance abusing patients. Knowledge of current community resources and continuum of care. Ability to interact with early recovery individuals and families. Excellent written & oral communication skills.

Description: To perform all duties related to the preparation of meal service items according to standard operating procedures.
Qualifications: H.S. Diploma, minimum of 2 years in food service, institutional cooking preferred.
